Disinfectant Selection

To select the appropriate disinfectant, the required disinfection intensity must first be determined based on the instrument category (Table 12-1).

High-level disinfection of the ultrasound transducer and use of sterile gel and transducer covers, as described in the instructions provided with the probe cover, is an acceptable method of ultrasound transducer infection control. Please refer to the FDA guidance document of September 30, 1997, Information for Manufacturers Seeking Marketing Clearance of Diagnostic Ultrasound Systems and Transducers.

General Cleaning Steps for All Probes

These general cleaning instructions apply to all probes. It is important to clean the probe and cable according to the following steps.

Warning

Always wear protective eyewear and gloves when cleaning and disinfecting any instrument.

Caution

The probe must be cleaned after each use. Cleaning the probe is an important step before effective disinfection or sterilization. When using disinfectants, always follow the manufacturer's instructions.

Do not allow sharp objects, such as scalpels or cautery knives, to come into contact with the probe or cable.

Do not allow the probe to impact hard surfaces when handling it.

Never use a surgeon's brush when cleaning the probe. Even a soft brush can damage the probe.

During cleaning, disinfection, and sterilization, ensure that the parts of the probe that must remain dry are positioned higher than the wet parts until all parts are dry. This will help prevent liquid from entering the unsealed areas of the probe.

Ensure that an appropriate concentration of enzymatic cleaner is used, and then rinse thoroughly.

To Clean the Probe

1. Wipe off the ultrasound transmission gel from the probe after each patient examination.

2. Disconnect the probe from the diagnostic system, and then remove any probe covers, biopsy guides, or end caps.

3. Use a soft cloth moistened with warm soapy water or enzymatic cleaner (according to the manufacturer's instructions) to remove particulate matter or body fluids adhering to the probe or cable. The pH of the enzymatic cleaner should be between 6.0 and 8.0. These cleaners are further diluted when used. For a list of approved enzymatic cleaners, see Table 12-2.

4. To remove adhering particulate matter and cleaning residue, thoroughly rinse the portion below the immersion point with water (Figure 12-3).

5. Wipe dry with a dry cloth.

Disinfecting Probes Using Wiping and Spraying Methods (Low-Level and Intermediate-Level Disinfection)

To disinfect the probe, use a disinfectant recommended by Philips Ultrasound, using either immersion or wiping methods. Use a biologically appropriate method as indicated in Table 12-1.

This section provides instructions on using the wiping or spraying method. For information on the immersion method, see "Disinfecting Probes Using the Immersion Method (High-Level Disinfection)".

Instructions

The wiping method can only be used to disinfect the probe if the product label of the disinfectant used indicates that the product can be used with the wiping method.

Limit the use of 70% isopropyl alcohol (rubbing alcohol) on the probe (Figure 12-1). Only use isopropyl alcohol solution to wipe the probe tip area beyond 2.5 cm (1 inch) from the strain relief/boot connector. Do not use isopropyl alcohol solution on the strain relief/boot connector, strain relief, cable, or TEE probe (except for the handle). Isopropyl alcohol solution may damage these probe components. Such damage is not covered by warranty or service contracts.

To Disinfect Probes Using Wiping or Spraying Methods

1. After cleaning the probe and cable (see "General Cleaning Steps for All Probes"), wipe or spray the probe and cable with a low-level disinfectant. Allow disinfection for up to the contact time recommended by the manufacturer. For a list of applicable disinfectants, see Table 12-2.

2. Remove any residue with a soft cloth moistened with water. Do not allow any disinfectant to air dry on the probe.

3. Inspect the instrument and cable for damage, such as cracks, crevices, sharp edges, or protrusions. If the instrument shows significant damage, discontinue use and contact your Philips Ultrasound representative.

Cleaning and Disinfecting Cables and Connectors Using Wiping and Spraying Methods

The cables and connectors of all probes can be disinfected using the recommended wiping or spray disinfectants. To protect the electronics in the connectors, Philips recommends using Philips connector seals when disinfecting around the connectors.

Using any other method than those recommended in this document to disinfect the connectors may damage the instrument and void the product warranty.

Be sure to position the parts that must remain dry higher than the wet parts until all parts are dry.

To Clean and Disinfect Cables and Connectors

1. Disconnect the instrument from the system.

2. Adjust the orientation of the instrument and connector so that they both face upward (Figure 12-2).

Do not allow any liquid to enter the connector. If liquid enters the connector, it may void the warranty of the probe or instrument.

Do not use a brush to scrub the connector label.

3. Clean the cable and connector with a soft cloth moistened with lukewarm soapy water or disinfectant. A soft brush can only be used to clean the metal surface of the connector. Do not allow any liquid to enter the device. Take care to prevent liquid from entering the device through the strain relief, through electrical contacts, or around the locking stem and strain relief (Figure 12-2).

If using premixed disinfectant, be sure to note its expiration date.

You may only use an alcohol solution to disinfect the connector. Ensure the alcohol content does not exceed 70%. If the alcohol content exceeds 70%, it may damage the instrument (Figure 12-1).

Do not use any alcohol or alcohol-based disinfectants on the cable.

Avoid contact between the disinfectant and the connector label.

4. Mix the disinfectant suitable for the cable (Table 12-2) according to the label instructions for disinfectant concentration.

5. Wipe or spray the cable and connector with disinfectant. Follow the disinfectant label instructions for wipe time, solution concentration, and contact time of the disinfectant with the cable. Ensure that the disinfectant concentration and contact time are suitable for the specific clinical use of the instrument. Ensure that the disinfectant does not enter the instrument or connector, or contact the connector label.

6. Air dry or wipe dry with a disinfected cloth, according to the instructions on the disinfectant label.

7. Inspect the instrument and cable for damage such as cracks, fissures, sharp edges, or protrusions. If the instrument shows visible damage, discontinue use and contact a Philips Ultrasound representative.

Disinfecting probes (high-level disinfection) by immersion

This section provides information on disinfecting most probes using immersion. Esophageal (TEE) probes require

separate disinfection by immersion. For more information, see "Disinfecting TEE probes by immersion".

If using premixed disinfectant, be sure to note its expiration date.

Using a non-recommended disinfectant, inappropriate disinfectant concentration, excessive immersion depth of the probe, or exceeding the recommended contact time may damage or discolor the probe and void the probe’s warranty.

Do not exceed the minimum time required for the disinfection level. See Table 12-1 for information on disinfection level requirements.

To disinfect a probe by immersion

1. Clean the probe according to "General cleaning steps for all probes".

2. Mix the disinfectant suitable for your probe (Table 12-2) according to the label instructions for disinfectant concentration. Disinfectants marked with a footnote "FDA 510(k) cleared" in Table 12-2 are those recommended for use in the USA.

3. Immerse the probe in the appropriate disinfectant as shown in Figure 12-3 for your probe. Follow the disinfectant label instructions to determine the immersion time for the probe. Do not exceed the minimum time required for the disinfection level.

4. Rinse the probe below the immersion point and air dry or wipe dry with a clean cloth, according to the disinfectant label instructions.

5. Inspect the probe for damage such as cracks, fissures, leaks, sharp edges, or protrusions. If the probe shows visible damage, discontinue use and contact a Philips Ultrasound representative.

Disinfecting TEE probes by immersion

Immersion disinfection is an accepted method for infection control of transesophageal probes. Philips recommends using protective probe covers during examination.

Disinfect the probe after receiving a new probe before performing the first examination. Clean and disinfect the probe immediately after each use to prevent the transmission of various pathogens to patients and others. Establish and prominently post a cleaning procedure that includes the following steps.

When using Cidex OPA disinfectant, if the manufacturer’s instructions are not carefully followed, residual disinfectant may adhere to the probe. Residual Cidex OPA on TEE probes may cause temporary staining of the mouth and lip areas and irritation or chemical burns to the mouth, throat, esophagus, and stomach.

To minimize the effects of residual Cidex OPA or other disinfectants,

Philips recommends the following measures:

Very carefully follow the disinfectant manufacturer’s instructions. For example, the manufacturer of Cidex OPA recommends rinsing the probe three times in fresh water.

Use protective probe covers during intracavity and TEE examinations.

Use sterile protective probe covers with sterile ultrasound transmission gel when performing biopsies and intraoperative examinations.

Limit the probe immersion time to the minimum time recommended by the disinfectant manufacturer (e.g., the manufacturer of Cidex OPA recommends a minimum time of 12 minutes).

To disinfect an intraoperative probe by immersion

1. Disconnect the probe from the system.

2. Remove all organic matter and other residue using the following methods:

a. Soak a gauze pad in mild soapy water. Do not use iodine-based soap.

b. Perform one of the following:

Wipe the gauze pad from the distal area and shaft to the control housing (deflector).

Use enzymatic cleaner to help remove residual protein. The pH of the enzymatic cleaner should be between 6.0 and 8.0. These cleaners are further diluted during use. Follow the manufacturer's instructions for dilution.

Do not rinse or immerse the control housing, cable, or connector.

3. Thoroughly rinse the distal end and shaft with water.

Do not bend the shaft into a circle with a diameter of less than 0.30 m (1 foot).

Do not use bleach on any TEE transducer.

Do not use harsh solvents such as isopropyl alcohol, acetone, freon, and other industrial cleaners on the transducer.

Do not soak the transducer for extended periods. Limit the transducer's immersion time in disinfectant to the minimum time recommended by the disinfectant manufacturer.

Do not rinse or immerse the connector or the portion of the cable near the connector.

Do not immerse or rinse the deflection mechanism.

Follow the disinfectant manufacturer's recommendations.

4. Disinfect the distal end and shaft in an appropriate disinfectant (Table 12-2).

5. Remove the distal end and shaft from the disinfectant and rinse thoroughly with water according to the disinfectant manufacturer's instructions.

6. Check the transducer for organic residue. If found, remove the organic matter and disinfect the transducer again.

7. Dry the distal end and shaft with a disinfected cloth or pad, or air dry.

The transducer deflection mechanism is not sealed. If disinfectant or other liquids Enter this deflection mechanism, it will corrode the gears and electrical connections. Such avoidable transducer damage is not covered by warranty or service contracts.

8. Gently wipe the deflection mechanism and handle with a gauze pad moistened with alcohol (70% isopropyl alcohol), or use T-Spray II to disinfect the handle and deflection mechanism according to the instructions.

9. Hang the transducer on a wall-mounted rack to air dry.

Never sterilize the transducer using an autoclave, ultraviolet radiation, gamma radiation, gas, steam, or high-temperature sterilization techniques. This will cause serious damage to the transducer. Such avoidable transducer damage is not covered by warranty or service contracts.Disinfecting TEE transducers with an automatic disinfector

The TD-100 is an automatic disinfector provided by PCI medical. For more information on this product,

can be found on the website www.pcimedical.com. This disinfector is only for those transducers listed as compatible solutions in the search results on the following TransducerCare Web (transducer maintenance website):

To clean and high-level disinfect TEE transducers with the TD-100 automatic TEE disinfector

b. Perform either of the following:

– Wipe from the distal end and shaft to the control housing (deflection mechanism) with a gauze pad.

– Use enzymatic cleaner to help remove residual protein. The pH of the enzymatic cleaner should be between 6.0 and 8.0. These cleaners are further diluted during use. Follow the manufacturer's instructions for dilution.

4. Disinfect the distal end and shaft with the TD-100 automatic TEE disinfector. Follow the manufacturer's instructions for disinfector operation.

5. Check the transducer for organic residue. If found, remove the organic matter and disinfect the transducer again.

6. Dry the distal end and shaft with a disinfected cloth or pad, or air dry.

7. Gently wipe the deflection mechanism and handle with a gauze pad moistened with alcohol (70% isopropyl alcohol), or use T-Spray II to disinfect the handle and deflection mechanism according to the instructions.

8. Hang the transducer on a wall-mounted rack to air dry.

Transducer disinfectionDisinfection is required if the instrument belongs to the category of strictly disinfected equipment and is used without a sterile protective sheath, or if the sterile protective sheath is broken.

Always wear safety glasses and gloves when cleaning, disinfecting, or sterilizing any instrument.

In neurosurgical applications, disinfected transducers should be used with pyrogen-free transducer sheaths.

If using premixed disinfectant, be sure to note its expiration date.

Only use disinfectants to disinfect the transducer. Using an autoclave, gas (EtO), or other disinfection methods not approved by Philips Ultrasound will damage the transducer and void your warranty.

Ensure that the disinfectant concentration and contact time are appropriate for disinfection. Always follow the manufacturer's instructions for use.

To disinfect the transducer

3. Immerse the transducer in disinfectant.

4. Follow the instructions on the disinfectant label for the required immersion time for transducer disinfection.

5. After reaching the recommended disinfection time, remove the transducer from the disinfectant.

6. Following the instructions on the disinfectant label, rinse the transducer below the immersion point with disinfected water and then air dry, or dry with a disinfected dry cloth.

7. Check the transducer for damage such as cracks, fissures, leaks, sharp edges, or protrusions. If the transducer shows significant damage, discontinue use and contact your Philips Ultrasound representative. Clean and disinfect the diagnostic system surface. The exterior surfaces of the diagnostic system can be disinfected using an appropriate disinfectant and a wiping method. Diagnostic system surfaces include the display screen, touch screen, and plastic and painted surfaces.

The TD-100 is an automatic disinfector provided by PCI medical. For more information on this product,

To clean and high-level disinfect TEE transducers with the TD-100 automatic TEE disinfector

b. Perform either of the following:

– Wipe from the distal end and shaft to the control housing (deflection mechanism) with a gauze pad.

– Use enzymatic cleaner to help remove residual protein. The pH of the enzymatic cleaner should be between 6.0 and 8.0. These cleaners are further diluted during use. Follow the manufacturer's instructions for dilution.

4. Disinfect the distal end and shaft with the TD-100 automatic TEE disinfector. Follow the manufacturer's instructions for disinfector operation.

5. Check the transducer for organic residue. If found, remove the organic matter and disinfect the transducer again.

6. Dry the distal end and shaft with a disinfected cloth or pad, or air dry.

7. Gently wipe the deflection mechanism and handle with a gauze pad moistened with alcohol (70% isopropyl alcohol), or use T-Spray II to disinfect the handle and deflection mechanism according to the instructions.

8. Hang the transducer on a wall-mounted rack to air dry.

The transducer deflection mechanism is not sealed. If disinfectant or other liquids Enter this deflection mechanism, it will corrode the gears and electrical connections. Such avoidable transducer damage is not covered by warranty or service contracts.

Never sterilize the transducer using an autoclave, ultraviolet radiation, gamma radiation, gas, steam, or high-temperature sterilization techniques. This will cause serious damage to the transducer. Such avoidable transducer damage is not covered by warranty or service contracts.

Transducer disinfectionDisinfection is required if the instrument belongs to the category of strictly disinfected equipment and is used without a sterile protective sheath, or if the sterile protective sheath is broken.

Always wear safety glasses and gloves when cleaning, disinfecting, or sterilizing any instrument.

In neurosurgical applications, disinfected transducers should be used with pyrogen-free transducer sheaths.

If using premixed disinfectant, be sure to note its expiration date.

Only use disinfectants to disinfect the transducer. Using an autoclave, gas (EtO), or other disinfection methods not approved by Philips Ultrasound will damage the transducer and void your warranty.

Ensure that the disinfectant concentration and contact time are appropriate for disinfection. Always follow the manufacturer's instructions for use.

3. Immerse the transducer in disinfectant.

4. Follow the instructions on the disinfectant label for the required immersion time for transducer disinfection.

5. After reaching the recommended disinfection time, remove the transducer from the disinfectant.

6. Following the instructions on the disinfectant label, rinse the transducer below the immersion point with disinfected water and then air dry, or dry with a disinfected dry cloth.

7. Check the transducer for damage such as cracks, fissures, leaks, sharp edges, or protrusions. If the transducer shows significant damage, discontinue use and contact your Philips Ultrasound representative. Clean and disinfect the diagnostic system surface. The exterior surfaces of the diagnostic system can be disinfected using an appropriate disinfectant and a wiping method. Diagnostic system surfaces include the display screen, touch screen, and plastic and painted surfaces.
